TMZ reports Tequila was scheduled to fly to Germany Wednesday night for an eight-city singing tour, but never made it to the airport.
Tila's manager was notified Thursday that his client was not on the flight. An assistant was sent to Tila’s home to investigate. No one answered the door. Now her rep is worried she's gone missing -- or worse.
Tequila was hospitalized with an aneurysm and entered rehab in March for substance abuse and psychological issues, following several suicide attempts, but was released in April with a clean bill of health.
Her girlfriend, Johnson and Johnson heiress Casey Johnson, 30, was found dead in 2010 of complications of untreated diabetes.
UPDATE: Tila is okay. Police told TMZ they went inside the home and emerged a few minutes after speaking with Tila. One officer told us, "I don't know why we were called here."

Casey Johnson's fiancee Tila Tequila tells "Extra" she will seek custody of Johnson's adopted daughter Ava.
Johnson's mother has custody of Ava and Tequila says that's not what Casey wanted.
"Her last wish was to have Ava, have me have Ava," she revealed. The former reality TV star says she will file papers to adopt.
Tequila was also not given the details of Johnson's funeral and was unable to see the heiress before she was buried.
"She's not resting in peace right now. She has come to me in my dreams," Tila explains.
Johnson, 30, died Jan. 4 and the cause of death has not been determined, but Tequila knows she would not purposefully take her own life. Tequila confesses, "I know for a fact, because Casey was so excited for our life together, I know for a fact it was not suicide."
She continued, "I know it was an accident because she did have a sleeping problem." Johnson suffered from diabetes and Tequila said she did not have trouble managing her disease.

Heiress Casey Johnson has been laid to rest at her family's estate in New Jersey.
"Today was just a private, open-casket funeral for immediate family," a source tells Us Weekly. A larger memorial for friends is reportedly scheduled in the next few days.
According to People magazine, Nicky Hilton, Casey's longtime friend and godmother of her daughter Ava, attended the quiet service.
Over the weekend, Johnson's reported fiancée Tila Tequila complained about being left out of memorial plans, saying, "I don't want to talk about the funeral. I wasn't allowed to go, and I am so upset. I can't sleep, and I am on suicide watch."
Johnson's pals have dissed the "Shot at Love" star for seeking attention from the media about Casey's death.
Johnson was found dead in a Los Angeles home on January 4. The cause of death has yet to be determined.

A source close to Tequila tells UsMagazine.com, "She wants so badly to go to the funeral, but nobody will give her the details or tell her anything. She is extremely frustrated, as what she shared with Casey was real and special. Nobody understood."
Over the weekend Tila, 28, told E! News, "I don't want to talk about the funeral. I wasn't allowed to go, and I am so upset. I can't sleep, and I am on suicide watch."
It seems the reality TV star has been seeking media attention via Twitter and photographers since the death of Johnson on Jan. 4.

Casey Johnson's death at 30 was untimely, but it didn't come as a shock to people in her inner circle.
"She had so many ups and downs, but lately, it wasn't very good," a friend of Casey's tells People magazine. "She tried rehab, wanted things to change... but it didn't stick."
The pal says that Johnson's increasingly bizarre behavior continued up until her death. "Just a few months ago, she was living like a crazy person," the source says. "Super erratic behavior. People could barely be around her."
The source says the Johnson family was estranged from Casey and had cut her money supply in hopes that she would turn her life around. "Her family couldn't handle her behavior and lack of respect, so they cut her off," the pal says. "They thought it would teach her to turn things around."
Paris Hilton and Nicole Richie, childhood pals of Johnson, reunited at the L.A. home of Nicky Hilton last night to mourn Casey's death.

Casey was reportedly engaged to reality star Tila Tequila when she died, and a close pal of Johnson, English model Jasmine Lennard, slams the "Shot at Love" star for enabling Casey's downward spiral.
Lennard tells Radar Online, "Casey's family tried desperately to help her... when it came out that Casey had been arrested, she had nobody and was lost. Tila got a hold of her and said, 'Hey, come out with me and we'll show them!' For her to die with this reputation, it's just not her. Tila is a sick, sick girl."
Johnson's autopsy was completed yesterday, but results of toxicology tests could take weeks.

Casey Johnson, an heiress to the Johnson & Johnson fortune and Tila Tequila's reported fiancée, was found dead this morning. She was 30.
Los Angeles Police Sgt. Charles Halliburton tells People magazine, "I can confirm there's been a death and the body has been identified as Casey Johnson." Casey's body was discovered in L.A. this morning.
Tila broke the sad news on her Twitter page, writing, "Everyone please pray 4 my Wifey Casey Johnson. She has passed away. Thank u for all ur love and support but I will be offline to be w family"
A rep from the L.A. Coroner's office confirms to "Extra" that they responded to a call "regarding a deceased female, last name Johnson." An autopsy will be conducted to determine the cause of her death.
Casey was arrested in Los Angeles this past November 30th on charges of grand theft of various items from the home of a friend. She was released on $20,000 bail and was awaiting trial.
She was the great-great granddaughter of a co-founder of the Johnson & Johnson company, and the daughter of New York Jets owner Robert Wood Johnson and socialite Sale Johnson.
She leaves a daughter, Ava, 4, whom she adopted from Kazakhstan in 2007.
