Report: Demi Moore Rushed to Hospital

Actress Demi Moore was taken to the hospital Monday night, says TMZ.

Law enforcement told TMZ a 911 call was placed at 10:45 PM and paramedics responded, arriving to Moore's L.A. home. After an examination, she was transported to a local hospital.

The "Margin Call" actress' rep told "Extra," "Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health. She looks forward to getting well and is grateful for the support of her family and friends."

Moore and husband Ashton Kutcher announced their separation late last year.
